Transportation: VCH Volunteer Medical Escort Program – volunteers accompany frail clients via HandyDart or taxi to and from medical appointments
Outreach:
home visits for Physiotherapy, Occupational therapy, Case Management, Nursing, referrals to home support services
Education & Workshops:
VCH Caregiver Support Program – caregiver education series, workshops, support groups, telephone support and information
Peer Support:
VCH Volunteer Program – Neighbourhood Connectors, Wellness Walkers, Leisure Buddies, one to one volunteer visits to clients in home/community setting
Food Security
:Shop by Phone – volunteer-run grocery shopping service for those unable to get out to do their own shopping
Language Specific: numerous languages spoken by health/service providers and/or will arrange interpretation services as required
Housing:
Subsidized Residential Care and Assisted Living – case manager will determine if eligibility criteria met and make referral
