Meals to You – subsidized meals to high nutritional risk, low income, home bound clients who are unable to access other nutritional programs/resources; Shop by Phone – volunteer-run grocery shopping service for those unable to get out to do their own shopping

VCH onsite and outreach nurse practitioner, network homemaker’s assessment of frail seniors by staff then referred to services mentioned

morning health check, hospital bag medical information for paramedic and hospital

arrange for homemaker, food delivery, meals on wheels, Wednesday food distribution program (Required letter from staff)
